Insurance & Fees

Health Insurance : We gladly accept most health insurance, but payment schedule is vary by each plan. We are in- network at BCBS-HMO, BCBS-PPO, BCBS-POS, Cigna, United Health Care, Athena, Humana and Ambetter. If you are unsure of your insurance benefits, we will be happy to contact your insurance company to verify coverage. Your insurance will not pay until  your deductible is met. If you already met your deductible, you only pay your co-payment  or co-insurance. 

Traveler's Insurance : We accept most Traveler's insurance, but payment schedule is vary by each plan. Some plan allowed direct reimbursement to us, some plan is not. Usually, it has no deductible. We will contact your insurance company.

Auto (Car) Insurance : We accept all Auto Insurance. Auto insurance is usually not "Up-front" payment. You may use your "Medical payment" or be hold your payment by "lawyer's lean".  The Auto Insurance plan is vary and complicated. We can give more detailed information by phone.

Affordable Payment Plan : We can even arrange for an affordable payment plan for your convenience. Please ask our associate about more details. *Please be noted that plan may not apply when insurance policy is applied.

Medicare, Medicaid, Veterans Care: Unfortunately, we are out-of network.
